On a new path and layer, create the path for the mouth. Fill a selection of the path with black, contracting the selection by 1 pixel and then filling it with the same light yellow. Use the line shape tool, with a pixel width of 2, to create the teeth outline.

Rasterize the line shape by right clicking on it from the layers panel, and then use the rectangular marquee tool to slice off the right side of the line. This will sharpen up the line a bit. Duplicate the line a couple of times, and then create 2 more paths for the horizontal lines of the teeth. Set the brush to 2 pixels hard and the right click on the horizonal line paths, chosing stoke path.
